completado&nbsp; pelo&nbsp; menos&nbsp; um&nbsp; quinqu\u00eanio&nbsp; (cinco&nbsp; anos)&nbsp; de&nbsp; efetivo&nbsp; exerc\u00edcio&nbsp; de servi\u00e7o p\u00fablico federal, sem interrup\u00e7\u00f5es, at\u00e9 15\/10\/1996.<\/p>\n<p style=\"text-align: justify\"><span style=\"color: #008000\"><strong>Documenta\u00e7\u00e3o necess\u00e1ria para instruir o processo<\/strong><\/span><br \/>\n1. &nbsp;Formul\u00e1rio de requerimento&nbsp;da Licen\u00e7a pr\u00eamio.<\/p>\n<p style=\"text-align: justify\"><span style=\"color: #008000\"><strong>Formul\u00e1rios<\/strong><\/span><br \/>\n<span style=\"color: #0000ff\"><strong><span style=\"text-decoration: underline\"><span style=\"text-decoration: underline\"><a href=\"http:\/\/www2.unifap.br\/drh\/files\/2010\/04\/Requerimento-para-Lic%C3%AAn%C3%A7a-Pr%C3%AAmio.pdf\" target=\"_blank\" rel=\"noopener\"><span style=\"color: #0000ff;text-decoration: underline\">Formul\u00e1rio de requerimento&nbsp;<\/span><\/a><\/span><\/span><\/strong><\/span><\/p>\n<p style=\"text-align: justify\"><span style=\"color: #008000\"><strong>Setor respons\u00e1vel<\/strong><\/span><br \/>\nDepartamento de Administra\u00e7\u00e3o de Pessoal &#8211; A Licen\u00e7a Pr\u00eamio por Assiduidade foi extinta em face da nova reda\u00e7\u00e3o dada&nbsp; ao&nbsp; Art.&nbsp; 87&nbsp; da&nbsp; Lei&nbsp; n\u00ba&nbsp; 8.112\/90,&nbsp; pela&nbsp; Medida&nbsp; Provis\u00f3ria&nbsp; n\u00ba&nbsp; 1.522\/96, passando para Licen\u00e7a Capacita\u00e7\u00e3o.<br \/>\n2.&nbsp; \u00c9 assegurada pela Instru\u00e7\u00e3o Normativa n\u00ba 12, a concess\u00e3o da licen\u00e7a relativamente&nbsp; aos&nbsp; q\u00fcinq\u00fc\u00eanios&nbsp; j\u00e1&nbsp; completados&nbsp; at\u00e9&nbsp; 15\/10\/96&nbsp; para&nbsp; efeito&nbsp; de gozo, ou convers\u00e3o em pec\u00fania no caso espec\u00edfico de falecimento do servidor, observando a legisla\u00e7\u00e3o anteriormente vigente.<br \/>\n3.&nbsp; Para&nbsp; o&nbsp; servidor&nbsp; que&nbsp; n\u00e3o&nbsp; completou&nbsp; quinqu\u00eanio&nbsp; (5 anos)&nbsp; de&nbsp; efetivo exerc\u00edcio at\u00e9 a data de 15\/10\/96, n\u00e3o haver\u00e1 o direito do usufruto de 3 meses para&nbsp; Licen\u00e7a-Pr\u00eamio&nbsp; por&nbsp; Assiduidade&nbsp; e&nbsp; sim&nbsp; para&nbsp; Licen\u00e7a&nbsp; para&nbsp; Capacita\u00e7\u00e3o (vide <a href=\"http:\/\/www2.unifap.br\/drh\/files\/2016\/05\/Requerimento-para-Licen%C3%A7a-Capacita%C3%A7%C3%A3o.pdf\" target=\"_blank\" rel=\"noopener\"><strong><span style=\"color: #0000ff\">LICEN\u00c7A CAPACITA\u00c7\u00c3O<\/span><\/strong><\/a>).<br \/>\n4.&nbsp; Considera-se&nbsp; efetivo&nbsp; exerc\u00edcio,&nbsp; para&nbsp; fins&nbsp; de&nbsp; concess\u00e3o&nbsp; de&nbsp; Licen\u00e7a-Pr\u00eamio,&nbsp; o&nbsp; tempo&nbsp; apurado&nbsp; na&nbsp; forma&nbsp; do&nbsp; disposto&nbsp; nos&nbsp; arts.&nbsp; 15&nbsp; e&nbsp; 102&nbsp; da&nbsp; Lei&nbsp; n\u00ba 8.112\/90.<br \/>\n5.&nbsp; Em&nbsp; caso&nbsp; de&nbsp; acumula\u00e7\u00e3o&nbsp; de&nbsp; cargos&nbsp; na&nbsp; mesma&nbsp; institui\u00e7\u00e3o,&nbsp; a&nbsp; Licen\u00e7a-Pr\u00eamio ser\u00e1 concedida em rela\u00e7\u00e3o a cada um deles.<br \/>\n6.&nbsp; Implicam&nbsp; em&nbsp; nova&nbsp; contagem&nbsp; do&nbsp; interst\u00edcio,&nbsp; n\u00e3o&nbsp; se&nbsp; considerando&nbsp; o per\u00edodo&nbsp; anterior:&nbsp; sofrer&nbsp; penalidade&nbsp; disciplinar&nbsp; de suspens\u00e3o,&nbsp; as&nbsp; licen\u00e7as&nbsp; por motivo de doen\u00e7a em pessoa da fam\u00edlia, sem remunera\u00e7\u00e3o ou para tratar de interesses&nbsp; particulares,&nbsp; a&nbsp; condena\u00e7\u00e3o&nbsp; \u00e0&nbsp; pena&nbsp; privativa&nbsp; de&nbsp; liberdade e o afastamento para acompanhar c\u00f4njuge ou companheiro.<br \/>\n7.&nbsp; Os&nbsp; per\u00edodos&nbsp; de&nbsp; gozo&nbsp; de&nbsp; Licen\u00e7a-Pr\u00eamio&nbsp; s\u00e3o&nbsp; considerados&nbsp; como&nbsp; de efetivo&nbsp; exerc\u00edcio.<br \/>\n8.&nbsp; A&nbsp; Licen\u00e7a-Pr\u00eamio&nbsp; pode&nbsp; ser&nbsp; gozada&nbsp; em&nbsp; per\u00edodo&nbsp; \u00fanico&nbsp; ou&nbsp; em&nbsp; tr\u00eas per\u00edodos, sendo que nenhum desses per\u00edodos poder\u00e1 ser inferior a trinta dias.<br \/>\n9.&nbsp; Quando&nbsp; se&nbsp; tratar&nbsp; de&nbsp; mais&nbsp; de&nbsp; uma&nbsp; Licen\u00e7a-Pr\u00eamio, o&nbsp; servidor&nbsp; poder\u00e1 goz\u00e1-las&nbsp; em&nbsp; per\u00edodos&nbsp; consecutivos&nbsp; ou&nbsp; isolados, em per\u00edodos trimestrais ou mensais.<br \/>\n10.&nbsp; Por&nbsp; aus\u00eancia&nbsp; de&nbsp; previs\u00e3o&nbsp; legal,&nbsp; o&nbsp; gozo&nbsp; de&nbsp; Licen\u00e7a-Pr\u00eamio&nbsp; s\u00f3&nbsp; poder\u00e1 ser&nbsp; interrompido&nbsp; por&nbsp; motivo&nbsp; de&nbsp; calamidade&nbsp; p\u00fablica,&nbsp; como\u00e7\u00e3o&nbsp; interna, convoca\u00e7\u00e3o para j\u00fari, servi\u00e7o militar ou eleitoral ou por motivo de interesse da Administra\u00e7\u00e3o.<br \/>\n11.&nbsp; O afastamento por motivo de Licen\u00e7a-Pr\u00eamio implica na suspens\u00e3o do pagamento&nbsp; das&nbsp; gratifica\u00e7\u00f5es&nbsp; de&nbsp; insalubridade,&nbsp; periculosidade&nbsp; e&nbsp; raios&nbsp; X.<br \/>\n12.&nbsp; Os&nbsp; per\u00edodos&nbsp; de&nbsp; Licen\u00e7a-Pr\u00eamio&nbsp; j\u00e1&nbsp; adquiridos,&nbsp; e n\u00e3o&nbsp; gozados,&nbsp; pelo servidor que vier a falecer na ativa, ser\u00e3o convertidos em pec\u00fania a ser paga aos benefici\u00e1rios da pens\u00e3o.<br \/>\n13.&nbsp; Os per\u00edodos da Licen\u00e7a-Pr\u00eamio adquiridos at\u00e9 15\/10\/96 e n\u00e3o gozados poder\u00e3o ser contados em dobro para efeito de aposentadoria.<br \/>\n14.&nbsp; Para&nbsp; o&nbsp; gozo&nbsp; da&nbsp; Licen\u00e7a-Pr\u00eamio&nbsp; por&nbsp; Assiduidade, somente&nbsp; poder\u00e1&nbsp; ser permitido o afastamento de at\u00e9 1\/3 da lota\u00e7\u00e3o da respectiva unidade, devendo haver&nbsp; escala&nbsp; de&nbsp; forma&nbsp; a&nbsp; atender&nbsp; o&nbsp; interesse&nbsp; do&nbsp; servi\u00e7o.<br \/>\n15.&nbsp; A&nbsp; conveni\u00eancia&nbsp; do&nbsp; servi\u00e7o&nbsp; \u00e9&nbsp; o&nbsp; fator&nbsp; determinante&nbsp; para&nbsp; o&nbsp; afastamento do&nbsp; servidor,&nbsp; portanto,&nbsp; caber\u00e1&nbsp; \u00e0&nbsp; chefia&nbsp; imediata&nbsp; determinar&nbsp; em&nbsp; que&nbsp; per\u00edodo poder\u00e1 ocorrer o afastamento.<br \/>\n16.&nbsp; O servidor ocupante de cargo em comiss\u00e3o ou em exerc\u00edcio de fun\u00e7\u00e3o de confian\u00e7a, durante o gozo de Licen\u00e7a-Pr\u00eamio, perceber\u00e1 apenas a remunera\u00e7\u00e3o do cargo efetivo.<br \/>\n17. A comunica\u00e7\u00e3o do per\u00edodo de usufruto da licen\u00e7a pr\u00eamio dever\u00e1 ser encaminhada ao Departamento de Administra\u00e7\u00e3o de Pessoal &#8211; DAP\/PROGEP com anteced\u00eancia m\u00ednima de 30 (trinta) dias antes da data de in\u00edcio de gozo da licen\u00e7a.<\/p>\n<ul>\n<li><strong>Tipo:<\/strong>&nbsp;Of\u00edcio-Circular<\/li>\n<li><strong>N\u00famero:<\/strong>&nbsp;<a href=\"https:\/\/legis.sigepe.planejamento.gov.br\/legis\/detalhar\/14594\" target=\"_blank\" rel=\"noopener\">91-2018-MP<\/a> (Ressarcimento dos valores recebidos em raz\u00e3o da convers\u00e3o em pec\u00fania da licen\u00e7a-pr\u00eamio por assiduidade em caso de revers\u00e3o.)<\/li>\n<\/ul>\n<p><strong>Previs\u00e3o legal<\/strong><br \/>\n1.
